Summer Camp Related Forms

• Photo Permission Slip - Release to be signed by each Scout's Parent/Guardian, which allows the Mount Baker Council to use photos of Scouts in camp for promotional purposes.
• Camp Map— Where are things in camp?
• Troop Roster Form OR Troop Roster Form (Microsoft Excel) - Please fill out this form completely before arriving at camp.
• Medical Form - The Boy Scouts of America recommends that all youth and adult members have annual medical evaluations by a certified and licensed health-care provider. Complete information about the form is found at the beginning of the form.
• Provisional Camping Form OR Provisional Camping Form (Microsoft Word File) - For a Scout who will be attending a Mount Baker Council Camp without his unit. See Camp Leaders Guide for more information.
• All Units coming to camp, must bring an approved Tour Permit Form Local Tour Permit - Less than 500 miles each way. (requires two weeks to process)National Tour Permit - More than 500 miles each way. (requires one month to process)
